A Pain-Free Breakthrough in Pediatric Dentistry?
A recent study published in JAMA Pediatrics has made a significant breakthrough in pediatric dentistry: a liquid solution called silver diamine fluoride (SDF) can stop tooth decay without drilling, injections, or sedation. Researchers at the University of Michigan have found that SDF is effective in treating cavities and preventing further decay.
For decades, dentists have used SDF off-label to treat tooth sensitivity, but this latest research provides robust evidence needed for FDA approval. The Road to FDA Approval
The University of Michigan study generated the clinical evidence needed for the manufacturer to submit an application to the FDA for SDF approval as a dental caries drug. With over $12 million in funding from the NIH’s National Institute of Dental and Craniofacial Research, researchers were able to recruit 830 children under age 6 across three states – Michigan, New York, and Iowa.
The trial showed that 38% of treated baby teeth experienced no further decay after six months. This is a significant finding, especially given the high prevalence of tooth decay among young children in the United States (over 40%).
Expanding Access to Care
With FDA approval on the horizon, dentists may soon be able to prescribe SDF more widely. Amr Moursi, professor of pediatric dentistry at New York University College of Dentistry and co-principal investigator on the study, believes that this could lead to increased utilization by providers, enhanced payments by insurers, and improved product quality.
However, some experts remain skeptical about the benefits of SDF. They argue that its off-label use for decades has already led to widespread adoption without concrete evidence. Others point out that more research is needed to fully understand the long-term effects of SDF on tooth structure and overall oral health.
A Bridge or Long-Term Solution?
One of the most intriguing aspects of SDF is its potential to function as either a bridge treatment or a long-term solution for cavities. For some children, applying SDF every few months may control decay until their baby teeth fall out naturally. In adults, it could provide temporary protection until more extensive restorative procedures become feasible.
As researchers continue to refine and validate the use of SDF, several questions remain unanswered. Will insurance coverage expand to include SDF treatments?
