In a fatal crash in Texas, the driver manually overrode FSD by pressing the gas pedal to 100 percent.

On June 29, 44 year old Michael Butler was driving his Tesla Model 3 on a two lane road with a 30mph speed limit when it veered off course and crashed into a residential home.
